Other than the thousands of zombies that have taken over Fortune City, Chuck Greene must deal with various psychopaths, citizens who have snapped or otherwise developed immoral values. He will encounter them as he searches for Zombrex, rescues survivors and attempts to clear his name during cases.
A pair of twins that serve as bodyguards for TK as well as lovely co-hostesses for the show Terror is Reality. The pair presumably framed Chuck for the zombie outbreak and later take Rebecca Chang hostage.
Leon Bell was one of the contestants at the Slicecycles event at the beginning of the game. Leon used to be a huge fan of Chuck but now disowns him. Leon fights Chuck from the Silver Strip to the Platinum Strip on his signature Slicecycle.
Bibi Love is an aging pop singer who is clinging to her former glory. After the zombie outbreak in Fortune City causes the last of her fans to flee, she goes insane and captures several people. Using a bomb with a trigger that connects to her microphone, she threatens to blow up her hostages, if they try to leave before her show is over. She mistakes Chuck for a stagehand and makes numerous requests that Chuck has no choice but to comply with. You can kill her, but it will say across the screen 'Bibi Love has defected' and then it will list all of the survivors, including her, who dies and you won't get points.
Seymour Redding is a security officer who has taken the zombie outbreak in Fortune City as an opportunity to act as de facto sheriff. Using his new-found authority, he executes one survivor after another by hanging. He is seen with a masked captive.
Antoine Thomas is self-described as the king of French cuisine. He was going to be interviewed by a reporter for a high-profile food magazine, but the zombie outbreak in Fortune City destroys his chance at fame. He mistakes Chuck for the reporter and offers him the greatest meal made from "ingredients" nobody has dared to imagine.
"Slappy", whose real name is Brent Ernst, is the mascot for Kids' Choice Clothing and was scheduled to perform in a 3PM show located at an unknown area. He was planning to go on a first date with Louise Jameson, the girl in the other mascot costume who "loved him for who he was". Before the date, however, Louise is killed in the zombie outbreak. Brent now stalks the mall looking for the man he believes to be responsible, Chuck Greene, and believes that he really is Slappy.
Randy is an obese man in a fetish costume who wields a large chainsaw. He does not want to die a virgin so he kidnaps a bride and his own father and forces his father to marry them. When his father cries for help, Randy kills him and attempts to kill Chuck for ruining his big day.
Looters are scavengers who are taking advantage of the Fortune City outbreak by stealing from various stores and casinos. They are first seen as a trio robbing a supermarket and terrorizing a pharmacist. Although some looters are fightable psychopaths, others are vendors who sell combo weapons at various pawnshops. They use spray paint to stun Chuck sometimes, and if you are struck while attempting to get the paint out of your eyes, you will wake up in a bathroom, stripped of your clothes and without any items.
Carl Schliff is a mailman that has never failed at his job and plans to keep it that way, even during an outbreak. Chuck Greene spots Carl driving his mailcart, trying to deliver a package of Zombrex to anyone still alive in Royal Flush Plaza, because he can't continue on his route without a signature. As Carl starts to hand over the Zombrex to Chuck, he sees Chuck's signature. Realizing who he is, he withdraws the Zombrex and hands Chuck a bomb and flees. Chuck throws the bomb at Carl, blowing up his cart and Carl, who has kept a perfect record for years, attacks him.
Reed Wallbeck and his partner try to regain their status as magicians by performing a classic; cutting a person in half. Instead they kill the woman on the table and they go to find another "volunteer". Chuck sees this he shouts at them saying, "That's not magic, she's dead!" When Reed hears this, he thinks he is criticizing him and says that he can't do any better.
