The combo card is obtained after purchasing all fortunes ($1,200,000 total) offered by the fortune teller outside of One Little Duck Bingo. In Dead Rising 2: Off the Record, you get it by watching "Paddle Party Massacre" at 10:30pm in the movie theater.
This weapon has similar attack motions to both the Super Slicer and the Moose Head, also the TiR Moose helmet from Pounds of Flesh, with its ability to bang one's head and kill a zombie, and the charge attack.
The components can both be found in the steakhouse in the Food Court.
The PP amount given when the combo card is in the inventory is 666, which is the "Number Of The Beast", traditionally associated with the devil. The flames and the skull's horns are also traditional demonic images.
If worn, it shows up in cutscenes, similarly to the Beer Hat and the Super Slicer.
The charge attack can sometimes be performed without the combo card. If the attack button is held after Chuck performs a headbutt, he should start charging (this was tested independently, but failed to occur; possibly an intermittent glitch).
This weapon has been told to be very effective against Antoine Thomas.
It has been suggested that this weapon is a reference to the Wendigo, the American Folklore Cannibal that is part man part deer.
